Runaway semi truck crashes into home

Investigators are trying to determine if the accident was caused by human error or a mechanical failure.


Well to save time, “Human Error”!
WHY?

Every driver of a 6, 10, or 18 wheel must, at least if it is a union driver, hit the air braking system as the last thing before leaving the cab!


Removing the air from the breaks locks up the brakes!

The only other thing, although I have my doubts, would be 
(Too much water in the air tank).

Still human error because driver is responsible to bleed the tanks.

The system consists of service brakes, parking brakes, a control pedal, and an air storage tank.

A sudden loss of air pressure would result in full spring brake pressure immediately.

The system must be filled with air before the brakes will release.

Even though the brakes remain engaged at rest, as soon as the system reaches its operating pressure, and the brakes are ready to use.

First and most important reason for these heavy-duty vehicles to use Air brake instead of hydraulic brakes is the fact that the use of air allows multiple vehicle units to be coupled. This way all units have braking capability and can be controlled from the cabin.
